Driver Permit Request

Requirements to receive a Driver Permit Form per the State of Kentucky. 

1. Must be 15 years old or older (Updated 4/7/25)

House Bill 15 lowers the age to obtain a Kentucky driver’s permit from 16 to 15. The School Compliance Verification Form: KRS 159.051 for No Pass/No Drive has been updated to reflect this immediate change. The revised form is attached, please distribute to appropriate staff in the district.   

2.  The student must be passing 4 of 6 courses. At the time of the request and they are not passing they will have to wait for the next term (6 weeks) to request again. If the student requests at the beginning of the year, it will determine what their grades were at the end of the previous year. 

3.  Student must not have more than 9 unexcused absences. 

Remember if the student receives their permit and they start to slip on their grades or start to miss school the State of Kentucky will take the students permit away. 

Please allow 24-48 hrs for the form to be completed. Forms WILL NOT be complete without the request form filled out and forms WILL NOT be filled out the day of.
