The Work-Based Learning (WBL) Application is REQUIRED before enrolling and/or participating in a WBL experience.
There are two versions of the WBL Application available: the online form and the PDF.
*Online application requires JCPS student email login and password.
*PDF will be emailed to school official or printed & submitted in person.

The Medical Authorization is REQUIRED before enrolling and/or participating in a WBL experience.
Parents/Guardians may utilize the WBL Form Dropbox to submit completed Medical Authorization.

The Transportation Verification form is REQUIRED for all student drivers in WBL experiences. Parents/Guardians may utilize the WBL Form Dropbox to submit completed Transportation Verification.
The WBL Student Candidate Plan must be completed by school officials and finalized prior to the start of a WBL experience. Once completed, all parties (Student, Parent/Guardian, Employer, and School Officials) will review before signing their WBL Agreements. Students can review a blank WBL Plan for information purposes ONLY.
The WBL Student Agreement is REQUIRED before enrolling and/or participating in a WBL Experience. The Agreement will be signed AFTER reviewing the completed WBL Plan with school officials. Students may utilize the WBL Form Dropbox to submit signed WBL Agreement.
The WBL Hours Log is REQUIRED to be submitted for all WBL experiences except (Job Shadow and School-Based Enterprises). Students must submit the log to school officials no less than once a month. Students may utilize the WBL Form Dropbox to submit completed logs.
The Work-Based Learning Student Evaluation is REQUIRED upon completing a WBL experience. Students may utilize the WBL Form Dropbox to submit completed WBL Student Evaluation.
The WBL Thank You Email Template should be used by all WBL Students upon completion of placement. This email will be sent to the WBL Employer and includes links to the Employer Evaluation and Work Verification (if applicable).
The Work Verification is completed by EMPLOYERS ONLY. WBL Students can provide this form to employers to verify past work hours in a WBL experience. Employers can submit completed Work Verification to the WBL Form Dropbox.
The WBL Career Portfolio is REQUIRED for all Experience-Based Work and Outside Work Experience students. More information available on the Career Portfolio page.
The WBL Plan: Change Request is REQUIRED for any changes that need to be made to a current WBL Plan. This template should be completed and submitted to school officials.
Use this form to upload completed Work-Based Learning forms. DO NOT SUBMIT WBL APPLICATIONS IN THE DROPBOX. Students should complete the Online WBL Application prior to using the Dropbox.
